After enrolling, you'll schedule your intake executive physical, activate your secure member portal, and complete your intake paperwork online. This groundwork ensures your first visit can focus entirely on you.
At your intake appointment, your physician reviews your health history, current concerns, and goals in depth, then recommends personalized lab testing, which our team orders that same day. From that point forward, you have direct access to our care team and may schedule urgent or acute visits as needed.

Your intake appointment is a comprehensive, unhurried visit focused on understanding you. Your physician reviews your medical history, current concerns, lifestyle factors, and long-term goals through a conversation designed to build a complete picture of your health.
Together, you'll outline clear, realistic goals to focus on before your next visit. We encourage patients to reflect on their goals beforehand and bring a notebook to capture key insights and recommendations.
Ongoing Care & Support
Following your intake, you enter an ongoing, collaborative care relationship with our practice. Members are encouraged to schedule a comprehensive physical every six months, and may book follow-up visits in person, by telehealth, or by phone as often as needed. You can message your care team through the member portal at any time with questions, updates, or concerns. Our team is consistently available to support you throughout your health journey.
