After enrolling, you'll schedule your intake executive physical, activate your secure member portal, and complete your intake paperwork online. This groundwork ensures your first visit can focus entirely on you.
At your intake appointment, your physician reviews your health history, current concerns, and goals in depth, then recommends personalized lab testing, which our team orders that same day. From that point forward, you have direct access to our care team and may schedule urgent or acute visits as needed.
Your Intake Appointment
Your intake appointment is a comprehensive, unhurried visit focused on understanding you. Your physician reviews your medical history, current concerns, lifestyle factors, and long-term goals through a conversation designed to build a complete picture of your health.
Together, you'll outline clear, realistic goals to focus on before your next visit. We encourage patients to reflect on their goals beforehand and bring a notebook to capture key insights and recommendations.
Ongoing Care & Support
Following your intake, you enter an ongoing, collaborative care relationship with our practice. Members are encouraged to schedule a comprehensive physical every six months, and may book follow-up visits in person, by telehealth, or by phone as often as needed. You can message your care team through the member portal at any time with questions, updates, or concerns. Our team is consistently available to support you throughout your health journey.

Introduction Call
A complimentary 15 minute conversation to learn more about Professional Integrative Care, meet your provider, discuss your goals, and determine whether our approach is the right fit for you. This is an opportunity to ask questions and identify the care pathway that best aligns with your needs.
Integrative Consultation
$800
A one hour comprehensive consultation focused on your health, symptoms, lifestyle, nutrition, medications, supplements, and relevant labs or diagnostics. Your provider will develop recommendations based on your needs. This consultation does not establish Professional Integrative Care as your primary care provider.
Musculoskeletal Consultation
$500
A one hour evaluation for musculoskeletal concerns, including a comprehensive history and physical examination with ultrasound evaluation when appropriate. Your provider will review findings, discuss personalized recommendations, and determine whether an injection is appropriate and which type may best address your condition.
Platelet-Rich Plasma
$850
A one hour regenerative medicine appointment using platelet-rich plasma derived from your own blood. Your provider will evaluate your condition, discuss your goals, and determine whether PRP is appropriate for your treatment plan. When indicated, the procedure may be performed with ultrasound guidance, followed by personalized post procedure recommendations.
